About the Wedding Information Form

1. The Wedding Information Form enables you to provide us with information about your Marriage Service.
2. The Form is available for download as a Portable Document Format (PDF) file.
3. To view and/or download a PDF you will need the Adobe Reader program.
4. If you do not have a copy of Adobe Reader, you can download it free of charge here:
5. If you are not able to enter data into the PDF, the most likely reason is that you are using an older version of Adobe Reader.  You can download the latest version by clicking on the Adobe icon above.

How to Use the Form

1. If you are making a payment by cheque, please complete the form, print it and send the form with your cheque to the Parish Office at:

2. If you are making a payment by credit card, please complete the form including your credit card details and submit the form by Email by pressing the Submit by Email button at the bottom of the form.
3. If you are not making a payment at this time, please complete the form and submit the form by Email by pressing the Submit by Email button at the bottom of the form.
